Pre-registration
You must call the pre-registration office to provide contact and insurance information if you are not currently a patient at the UCLA Medical Center or a UCLA doctor has not seen you in more than a year. Please do this at least two days before your appointment.

current insurance cards
most recent pair of eyeglasses
New Patient Questionnaire
(download, complete and bring with you)

Payment for Services
Our office collects co-payments, deductibles and any charges for non-covered services on the date of service. Non-covered services may include:

refraction for glasses, $50 

If you would like to submit non-covered charges to your insurance, we will provide you with a receipt and billing sheet.
