During the preoperative evaluation, you will have the opportunity
to ask questions about your surgery. You will sign one or more consent
forms acknowledging that you have been informed about the risks
and benefits of surgery. Patients participating in a clinical trial
will review and sign an additional consent form specific to the
trial.

Trials
If your surgery will be performed in the main operating rooms of
the Jules Stein Eye Institute, you will need to have an internist
or general practitioner complete a Preoperative History and Physical
Examination form. If you have a significant cardiac history and
are under the care of a cardiologist, please have your cardiologist
complete the form. It is your responsibility to see that the form
is brought with you or faxed to the pre-anesthesia office. Do not
mail these forms. The pre-anesthesia staff will review the completed
form at the time of your preoperative evaluation. If you will be
having refractive surgery or a laser procedure, it will not be necessary
to have a preoperative history and physical examination.
