Pre-registration
You must call the pre-registration office to provide contact and
insurance information if you are not currently a patient at the UCLA
Medical Center or a UCLA doctor for has not seen you in more than a
year. Please do this at least two days before your appointment.

What you should bring
Please bring the following to your initial visit:
- Current insurance card and authorization (if
necessary)
- Most recent pair of eyeglass
- List of medications
Please be aware that although we provide some vision
services and can provide prescriptions for glasses, we do not offer
contact lens related services. If you require contact lens services, we
will refer you to the Contact Lens Department here at the Jules Stein
Eye Institute.
Payment for Services
Our office collects co-payments, deductibles and any charges for
non-covered services on the date of service. Non-covered services may
include:
- Refraction to measure eyeglass prescription, $50
- Corneal topography up to $180 per eye
- Routine eye exams (unless covered by your insurance)
If you would like to submit non-covered charges to your
insurance, we
will provide you with a receipt and billing sheet.
